Per Diem Nurse Assessor (Cantonese/Mandarin Speaking) (Healthcare)
Job Overview:
The role of the Per Diem Nurse Assessor is to be responsible for the assessment and enrollment of new members into the plan. You are also responsible for the reassessment of existing members. Nurse Assessors function as member of care management team in execution of 6 month care management visit of existing enrollees. Employee responsibility pertains to required duties at Hamaspik Choice and/or by controlling entities/individuals as necessary.
Responsibilities
- Conducts UAS-NY assessment visits for potential plan members and UAS-NY reassessment visits.
- Conducts assessment of newly referred members, assesses overall health needs and eligibility for enrollment.
- Provides member and family teaching at initial and reassessment visits.
- Post-visit communication with intake/enrollment related to member's eligibility status and with members of care management and care planning teams.
- Initiates development and coordination of a safe plan of care with member/family to meet all members' needs; coordinates with rest of care management team.
- Documentation of UAS-NY and visit notes including progress note and plan of care.
- Communication of members' needs and identified care plan to members' care manager.
- Reassessment of members' needs at 6 month reassessment visit; initiation of updated plan of care as appropriate.
- Attends professional and required in-service programs and staff meeting.
- Participates in the agency QA Program, as requested.
- Is available to cover shifts for other nurses as necessary and reasonably able.
- Is available on weekends/holidays/evenings by phone to provide on-call coverage as necessary and reasonably able.
- Performs assessments and completes documentation within the time frames set forth by the Agency.
- Participates in mandatory staff meetings.
- Other assignments within the RN Practice Act.
- Knowledge of member assessment procedure.
- Completion of UAS-NY training courses.
- Knowledge of member teaching process.
- Knowledge of pathophysiology.
- Knowledge of medication, administration, via oral, IM and SQ.
- Knowledge of medication interactions (drug-drug, drug-food).
- Knowledge of the principles of therapeutic diets.
- Knowledge of infection control practices, aseptic techniques.
- Knowledge of catheter insertion and care.
- Demonstrates ability to effectively communicate information with members of all ages and socioeconomic levels.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
- Ability to speak effectively before members or employees of the agency.
- NYS Licensed and currently registered as a Registered Professional Nurse.
- 2 years satisfactory office experience; experience in Managed Care, desired.
- This job description is not all inclusive and the employee may be asked to assume additional responsibilities.
- Have the capacity to do at least 30 assessments a month and in person throughout Brooklyn.
