We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Sands Point Center for Health & Rehabilitation, a premier healthcare facility located in Port Washington, New York. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission to deliver exceptional healthcare services.
Job SummaryAs a Licensed Practical Nurse, you will be responsible for providing direct patient care and supporting our interdisciplinary team in the delivery of patient-centered care. You will assess, plan, implement, and evaluate care based on age-specific components, and collaborate with other healthcare professionals to ensure quality patient outcomes.
Responsibilities- Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care
- Assesses, plans, implements and evaluates care based on age-specific components
- Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction
- Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences
- Administers medications and procedures per established policies and guidelines
Influences care outcomes by collaborating with members of the interdisciplinary team
- Must have an Associate's degree in Nursing (ADN) or equivalent
- Current LPN license in the state of New York
- Minimum of 0 years of experience, with new grads and LPNs with limited permit welcome to apply
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong communication and interpersonal skills
