Adult Geriatric Nurse Practitioner

• Provide primary care services as an Adult Geriatric Nurse Practitioner as part of a multi-disciplinary team.
• Conduct patient examinations, order and interpret diagnostic tests, and analyze records and results.
• Prevent, diagnose, and treat illness, disease, or injury; prescribe and administer medications, therapies, and vaccinations.
• Explain procedures, test results, and treatment plans to patients.
• Manage laboratory work, referrals, and coordinate with physicians, nurse practitioners, and support staff.
• Perform additional job-related duties as required by departmental needs.

• Completion of an approved/accredited Nurse Practitioner educational program.
• Valid New York State license, CDS, DEA, and BLS certifications.
• Effective communication skills and adherence to established procedures and protocols.
• Understanding of and passion for community healthcare (preferred).
