Position: NURSE RESIDENT
Location: New York

Job Purpose

The Nurse Residency Program (RN) is a structured transition-to-practice program designed to support newly licensed Registered Nurses as they develop the clinical competence, confidence, and professional judgment necessary to deliver high-quality care within the Program of All-Inclusive Care for the Elderly (PACE) care model. Through a combination of hands‑on clinical experience, mentorship, classroom and field training, and interdisciplinary collaboration, Nurse Residents will gain exposure to home‑based care, geriatric medicine, chronic disease management, and coordinated care delivery.

Under the guidance of experienced nursing leaders and clinical preceptors, Nurse Residents will progressively develop the skills required to provide compassionate, patient-centered care to frail and elderly participants in both home and clinical settings. Upon successful completion of the residency program, participants may transition into a Regional Nurse or other clinical nursing roles within the organization based on performance and organizational needs.

Job Responsibilities

Clinical Training & Patient Care:

  • Deliver nursing care to participants in accordance with individualized care plans, physician orders, and evidence-based nursing practice standards.
  • Conduct supervised home visits to support clinical services including wound care management and dressing changes, medication administration and reconciliation, participant education and caregiver support.
  • Observe, assess, and document participant health conditions and report changes to the interdisciplinary care team and supervising nurse.
  • Support disease management and health promotion for participants with chronic conditions.

Clinical Learning & Professional Development:

  • Participate in clinical training modules focused on geriatric care, chronic disease management, medication safety, infection control, and community-based care delivery.
  • Receive ongoing mentorship and clinical guidance from experienced nurses and clinical leadership.
  • Participate in case reviews, clinical discussions, and interdisciplinary team meetings.
  • Develop competency in clinical documentation and use of electronic health record systems.

Interdisciplinary

Collaboration:

  • Work closely with physicians, nurse practitioners, social workers, rehabilitation therapists, and other interdisciplinary team members.
  • Support care coordination efforts to ensure participants receive comprehensive, high-quality care.
  • Participate in care planning discussions and team-based decision making.

Compliance & Documentation:

  • Maintain accurate and timely documentation in the organization's electronic health record system.
  • Adhere to all clinical protocols, regulatory requirements, and infection control standards.
  • Meet the competency standards established by the PACE organization prior to independent practice.
Program Structure
  • Structured clinical orientation and training curriculum.
  • Supervised clinical practice with experienced nurse preceptors.
  • Participation in interdisciplinary team meetings.
  • Ongoing clinical education sessions and skills development.
  • Progressive responsibility as clinical competencies are achieved.

Program length and structure may vary based on organizational needs and clinical competency progression.

Schedule:

8:30 AM – 5:30 PM. Weekly

Hours:

The Bronx, Westchester, Manhattan, Queens, Brooklyn, Long Island.

Qualifications
  • Education: Bachelor of Science in Nursing (BSN) from an accredited nursing program required. Candidates expected to graduate from a CCNE/NLN-accredited nursing program prior to program start date are encouraged to apply.
  • Experience: New graduate Registered Nurses or nurses with limited clinical experience are welcome. Previous clinical rotations or internships in geriatric, community health, home care, or long-term care settings preferred. Demonstrated interest in working with frail or elderly populations.
