Licensed Practical Nurse (LPN)

Samaritan Daytop Village  is a leading human services organization in New York State serving over 38,000 New Yorkers annually across more than 60 locations through Transitional Housing, Community Based Services, and Residential and Outpatient programs. First Steps Intensive Crisis Stabilization Center (ICSC),operated by Samaritan Daytop Village  in Rockland County, is a 24/7 program dually licensed by OMH and OASAS to provide rapid, short-term support for children ages 5+ and adultsin mental health or substance use crisis.

Our multidisciplinary teamutilizesa trauma-informed, harm-reduction framework to stabilize individuals and connect them with long-term care while avoiding unnecessary emergency room visits.

We are seeking a Licensed Practical Nurse (LPN) to provide essential clinical support and compassionate patient care. Working under the direction of Registered Nurses, Nurse Practitioners and Physicians, you will play a key role in medical stabilization, vital sign monitoring, and the daily health management of individuals experiencing acute mental health or substance use emergencies. This role requires a dedicated professional who can balance technical nursing duties such as wound care, medication coordination, and EHR documentation.

As a vital member of our 24/7 multidisciplinary team, you will ensure a safe, organized, and respectful environment for clients navigating the road to recovery.

Responsibilities
  • Conducts patient intake, takes medical histories, and completes data entry in the clinical system
  • Monitors vital signs, including blood pressure, pulse, temperature, and weight
  • Administers injections, performs clinical tests, and provides wound care as ordered
  • Follows physician, nurse practitioner, and RN orders to provide direct patient care and medication coordination
  • Documents detailed reports of care and maintains accurate charting in the Electronic Health Record (EHR)
  • Reports significant changes in a patient’s physical or mental status immediately to the RN or Physician
  • Communicates with pharmacies to place prescription orders while maintaining strict confidentiality
  • Reinforces clinical instructions given by the RN or Physician to ensure patient understanding
  • Utilizes therapeutic communication to assist patients and initiates emergency assistance as per facility policies
  • Manages clinical workspace, including stocking supplies and preparing patient charts for the team
  • Participates in staff development, nursing conferences, and ongoing clinical training
  • Collaborates with a multidisciplinary team to ensure safe and effective patient care
  • Maintains a neat work area and ensures patient rooms are fully equipped for daily operations
  • Engages with individuals from diverse backgrounds to explain medical instructions with clarity and empathy
Qualifications
  • Active, unrestricted New York State Licensed Practical Nurse (LPN) license
  • Current BLS certification
  • High School Diploma or GED and graduate of a formal Technical or Vocational LPN Program
  • Minimum of one (1) year of experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)
  • Available to work a flexible schedule as needed in response to participant and staff needs
  • Computer literacy including proficiency in Microsoft Office Suite and Electronic Health Records (EHR)
  • Knowledge of medical terminology and ability to multi-task with excellent organization skills
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds
  • Ability to work effectively both independently and as part of a multidisciplinary clinical team
Benefits

This is a full-time non-exempt role with an hourly rate of $27.47-$32.06 (annualized at 35 hours a week to $50,000-$58,350) with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match and tuition reimbursement.
