- Collaborating with residents, families, and interdisciplinary teams to develop personalized care plans
- Implementing care plans with a resident-centered focus
- Utilizing available resources for counseling and discharge planning
- Collaborating with healthcare professionals to adjust care plans based on changing needs
- Valid Registered Nurse (RN) license in New York
- Current CPR certification
- Basic computer skills
- Minimum 2 years of experience as a Registered Nurse (RN) in Skilled Nursing Facility or Nursing Home experience
- Charge Nurse, Unit Manager, or Supervisor experience
- Onboarding Documents: A photo , a copy of your nursing license, social security card, and bank account are required for this role
