Job Description

Performs an age‑specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment to coordinate patient care and direct/delegate activities of the patient care unit team.

  • Conducts patient interview, explains policies and procedures to patient/surrogate, reviews patient’s chart and answers questions courteously; assesses gastrointestinal, cardiovascular, respiratory, renal and neurological health status; determines mobility, sensory deficits, prostheses use, and skin condition and adjusts plan of care, as appropriate; assesses level of pain and pain management; documents patient’s physiologic health status and plan of care; assesses psychosocial health status;

    elicits perception of medical/nursing care and expectations of care.
  • Determines coping mechanism, knowledge level, and ability to comprehend; identifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction; documents psychological status and care plan of support groups, counseling services, and social services; identifies patient outcomes; develops criteria to measure patient outcomes; identifies actual/potential patient problems; identifies patient’s need for teaching based on psychosocial and developmental assessment.
  • Develops patient outcome statements and establishes individualized patient goals; identifies care activities and establishes priorities necessary to achieve expected outcome; coordinates cost‑effective use of supplies, equipment and medication; documents the plan of care and collaborates with physicians; implements the plan of care; maintains vigilance over patient care to ensure safety precautions are followed (side rails up, call lights and bedside stand within reach, etc.).
  • Exercises professional skills related to the plan of care; reassesses patients as needed and revises plan of care; administers prescribed treatments correctly; uses appropriate equipment; provides emotional support; applies scientific principles; carries out procedures safely, timely and efficiently; makes accurate observations; reports and records as appropriate; keeps accurate documentation of treatment, activity and condition; uses documentation methods according to policy; acts rapidly during emergencies;

    provides calm atmosphere; communicates effectively with family; participates in discharge planning; coordinates referrals; provides discharge instructions; evaluates care for outcome.
  • Demonstrates ability to measure effectiveness of care and documents results; performs variance analysis on outcome data; designs, implements, evaluates improvement systems; collaborates with care team; directs team members; collaborates with physicians on patient plan of care.
  • Accurately interprets and implements treatment regimen as prescribed by Physician; assists Physician during treatments/diagnostic procedures; keeps Patient Care Manager/physician abreast of changes; delegates assignments clinically; performs Decubitus Care; preventive skin care; applies simple dressings with aseptic technique.
  • Applies warm and cold compresses per procedure; performs irrigations; performs other skin care procedures as necessary; administers medications correctly and safely; identifies medication action, dosage, side effects; meets medication administration standards; demonstrates preparation of local solutions.
  • Administers and documents medication; educates patients and significant others about drug/food interactions; participates in patient/family education; provides explanation of condition; communicates assessment data orderly; utilizes teaching opportunities and resources; provides early discharge planning and referrals; evaluates effectiveness of teaching via feedback and documents findings.
