OverviewAs a Care Manager at New York-Presbyterian-Queens, you will be a highly valued leader from day one, empowering patients and families with comprehensive, autonomous, and compassionate care. Your role includes leading the full spectrum of patient care, from pre-admission to discharge, ensuring quality outcomes and optimal length of stay.
Responsibilities- Provide intensive care management for adult patients with complex medical needs.
- Serve as a liaison between the interdisciplinary team—including social workers, physicians, physical and occupational therapists, cardiologists, and intensivists—to meet patient and family needs.
- Direct all aspects of inpatient care, develop individualized care plans, and ensure optimal length of stay.
- Help prevent hospital readmission by anticipating outpatient service needs and coordinating appointments and consultations before discharge.
- Promote a collaborative relationship with medical staff to accomplish case-management objectives and reduce length of stay.
- Minimum 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting.
- Minimum 1 year of leadership experience in a healthcare setting.
- Bachelor’s degree in Nursing (BSN) and current New York State Registered Nurse (RN) licensure.
- Minimum 3 years clinical experience as an RN in a medical or surgical unit, home care, or post-acute facility.
- Case Management Certification (required).
- Master’s degree in a healthcare-related field (preferred).

CompensationSalary Range: $141,250 - $169,000 per year.
Other InformationThis role requires on-site presence; employees must live within a commutable distance to NYP. NYP does not reimburse travel expenses.
