About This Role

The RN Transplant Coordinator is a Registered Professional Nurse who coordinates clinical programs and services for designated patient populations within the Transplantation Institute, ensuring comprehensive patient care throughout the transplant process.

• Coordinate multidisciplinary screening and evaluation of patients in the transplant practice.
• Ensure all necessary lab tests are performed on potential transplant candidates.
• Participate in making recommendations regarding placing patients on the transplant waiting list.
• Communicate with all active transplant candidates regarding waiting list status.
• Facilitate organ offers from UNOS and communicate with surgeons and patients.
• Follow patients during hospitalization and coordinate outpatient follow-up visits.
• Educate patients and families on transplant medications and lifestyle changes.
• Assess patient recovery through compliance with outpatient visits and lab reviews.
• Collaborate with healthcare team in developing and reviewing policies and procedures.
• Participate in continuous quality improvement programs and risk management activities.

• NYS RN with BSN required.

• At least 2 years of acute care hospital nursing experience.
• Critical care experience preferred.

• Current NYS RN License required.
• Master's degree, certification, or experience working with the patient population preferred.
