In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Chemung County, Elmira, NY

Signify Health, part of CVS Health®, is seeking a Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations in the Chemung County, Elmira, NY area. You’ll engage with people in the comfort of their own homes, building trusted relationships and delivering personalized health evaluations.

Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the patient’s health plan (e.g., diabetic eye exams, spirometry). Patients are not required to prescribe medicine, order lab tests, or alter treatment regimens. You’ll use an iPad with an intuitive clinical workflow during each evaluation.

• Participate in the effort to visit millions of people nationwide, providing connections to the right care for people’s unique needs.
• Work as a clinician colleague with Signify Health, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including diagnostic and preventive services (e.g., spirometry, DEE, bone density screening).

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the following primary location:
  Chemung County, Elmira, NY.

Pay for this position is based on the number of In‑Home Health Evaluations completed. Base pay per visit ranges from $46.03 to $99.14, with additional reimbursement for completed diagnostic and preventative services. Pay is dependent on experience, education, geography, and other criteria.
