We are seeking a full-time Traveling Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide onsite psychiatric services across skilled nursing facilities throughout New York State. This role is ideal for a clinician who enjoys travel, variety, and working with diverse patient populations while making a meaningful impact on an underserved geriatric community. The position is fully onsite and best suited for a highly reliable, adaptable provider who is comfortable working in dynamic environments and responding to coverage needs as they arise.
Telehealth is utilized only in rare circumstances such as severe weather conditions.
- Prior nursing home or long-term care psychiatric experience required
- Strong clinical judgment and ability to work independently
- Reliable transportation and willingness to travel statewide
- Opportunity to travel across New York and work in multiple facility environments
- Exposure to new teams, workflows, and clinical settings on a regular basis

- Critical role in ensuring facilities have consistent psychiatric coverage when needed
- Conduct comprehensive psychiatric evaluations and ongoing follow-up visits
- Diagnose and treat psychiatric and behavioral health conditions in geriatric populations
- Prescribe and manage psychotropic medications and monitor treatment outcomes
- Develop individualized treatment plans in collaboration with interdisciplinary teams
- Work closely with nursing staff, social workers, medical directors, and facility leadership
- Travel reliably to assigned locations throughout New York State
- Provide short-notice coverage when operational needs arise
- Maintain accurate, timely, and compliant clinical documentation
- Deliver patient-centered, trauma-informed, and evidence-based care
- Provide essential psychiatric care to underserved geriatric populations
- Improve quality of life and clinical stability for long-term care residents
- Reduce unnecessary hospitalizations through consistent psychiatric management
- All services delivered onsite at skilled nursing facilities
- Travel is a core requirement of the role
