- Observe and document resident changes in condition accurately per policy
- Ensure continuity of care by providing direct care and supervising licensed practical nurses, nursing assistants, and nursing students
- Monitor, treat, and report resident pain utilizing pain management techniques
- Administer medications following medication rights to reduce medication errors
- Give thorough reports to oncoming shifts by documenting in resident records and 24-hour reports
- Assist with medical procedures as needed
- Maintain role as resident advocate with a focus on the mission of the facility
- Implement plan of care based on assessments and goals established by the interdisciplinary care team
- Ensure residents at end of life or on hospice services are kept clean, dry, and comfortable
- Educate patients and family members on treatment and care plans, and answer their questions
- Current and valid RN License in New York State
