• Provide age‑appropriate care and intervention for patients, including assessments and treatment plans.
• Assist with patient assessments and support services.
• Develop and periodically review collaborative practice agreements.
• Assess health status via comprehensive history and physical examinations.
• Initiate therapeutic and diagnostic procedures, including lab orders and medication adjustments per collaborative agreement.
• Deliver teaching and counseling on health promotion, disease prevention, and medication management.
• Document findings and plans in the patient medical record.
• Consult with physicians as established in the collaborative agreement.
• Initiate referrals, admissions, and discharges in collaboration with physicians.
• Work collaboratively with interdisciplinary teams to promote optimal patient outcomes.
• Demonstrate professionalism and accountability in nursing practice.
• Advocate for patients and support patient rights.
• Serve as a resource for nurses and team members.
• Participate in peer review, research studies, and quality improvement initiatives.
• Contribute to maintaining a positive image of professional nursing and the organization.
• Participate in departmental and interdisciplinary committees influencing nursing practice policies.
• Maintain departmental policies, quality assurance, safety, environmental, and infection control standards.
• Perform duties according to the collaborative physician agreement with endorsement by senior leadership.
• Keep current knowledge via self‑directed learning and educational programs.
• Perform other related duties as required.

• Completion of an accredited Nurse Practitioner program with current NYS licensure and registration.
• Valid DEA number.

• Experience managing TAVR patients.
• National certification as an Acute Care Nurse Practitioner.
• Knowledge of advanced hemodynamic monitoring and cardiac assist devices.
• Experience in program development.
• Minimum of 3 years of clinical experience in cardiology, heart failure, or structural heart programs.
