We are hiring a full-time Hospice RN Case Manager to join our North Brooklyn hospice team.
This is a field/community-based RN role supporting hospice patients and families in a focused North Brooklyn territory. The RN will not be responsible for covering the entire borough. Territory assignments are clustered by nearby zip codes to help reduce unnecessary travel and create a more manageable day.

Schedule:
Monday through Friday, 8:30 AM – 5:00 PM
Approximately 4 hospice visits per day
Patient visits are typically completed earlier in the day. Charting, care planning, and coordination calls can often be completed from home in the afternoon.
This role is ideal for an RN who wants meaningful hospice work, a manageable daily visit expectation, strong benefits, and the ability to provide thoughtful end-of-life care without feeling like they are constantly running across the entire city.
Responsibilities- Manage an assigned hospice caseload of terminally ill patients
- Provide skilled nursing care in home and facility-based settings
- Support patients and families throughout the end-of-life process
- Develop and update individualized hospice plans of care
- Complete physical, psychosocial, and symptom-management assessments
- Establish patient-centered goals of care based on patient and family needs
- Educate patients and families on disease progression, medication administration, and symptom management
- Collaborate with physicians and the interdisciplinary hospice team
- Coordinate services outlined in the hospice plan of care
- Assist with medication titration and clinical care updates as appropriate
- Assess need for medical equipment, supplies, and safe use in the home
- Coordinate with office staff and vendors for timely equipment and supply delivery
- Anticipate supply needs before weekends and holidays
- Follow infection-control standards and organizational policies
- Serve as a clinical resource and liaison for patients in crisis when needed
- Maintain accurate, timely clinical documentation
- Active New York State RN license in good standing
- At least 1 year of RN experience required
- Hospice, palliative care, med/surg, or oncology experience preferred
- BSN strongly preferred
- Comfortable working independently in the field
- Strong communication, organization, and patient/family education skills
- Passion for hospice, palliative care, and end-of-life support
