Nurse Practitioner​/Physician Assistant

Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Capital Care Family Practice Clifton Park, a well‑established primary care practice providing comprehensive care to patients of all ages and committed to coordinated, high‑quality, patient‑focused care.

As an Advanced Practitioner at Capital Care Family Practice Clifton Park, you will deliver full‑spectrum family medicine services in a collaborative, team‑based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling designed to promote work‑life balance and long‑term professional satisfaction.

• Conduct comprehensive patient evaluations and manage acute and chronic conditions
• Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed
• Provide preventive care, health maintenance, and patient education
• Collaborate closely with physicians, nurses, and support staff to ensure continuity of care
• Maintain accurate, timely clinical documentation in accordance with organizational standards

• Care across the lifespan – serving pediatric, adult, and geriatric populations
• Structured five or four‑day schedule available (8:00 AM – 4:30 PM) with a consistent daily workflow and protected midday administrative/lunch time
• Telemedicine visits and secure patient portal, enhancing access and continuity of care
• Established, team‑based environment with multiple physicians and Advanced Practitioners supporting a strong patient panel
• Convenient Clifton Park location, serving a growing suburban community in the Capital Region

Starting at $125,000 for full‑time. CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
