In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Erie​/Buffalo, NY

About the Role

Signify Health, a part of CVS Health®, seeks a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In‑Home Health Evaluations. Clinicians visit patients in their homes, review medications and medical history, perform a physical examination, and, if indicated by the patient’s health plan, order diagnostic tests such as diabetic eye exams and spirometry. Clinicians use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine or alter treatment regimens.

• Visit lasts about 45 minutes, allowing time for one‑on‑one interaction and health education.
• Use an iPad and clinical workflow; no prescribing or laboratory ordering required.
• Work may involve local travel and travel within licensed states; virtual visits may also be required.

• Provide in‑home health evaluations and build trusted relationships with patients.
• Conduct medication and medical history reviews, physical evaluations, and diagnostic or preventive services (spirometry, DEE, bone density screening, etc.).
• Answer patient questions and provide education to improve health outcomes.
• Meet Clinician role requirements set by Signify Health and comply with state and company standards.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required; multi‑state licenses preferred.
• Experience as a Nurse Practitioner or Physician Assistant.

Typical pay range: $46.03 – $99.14 per hour. Actual offer depends on experience, education, geography, and other factors.
