Job Overview

Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and continuity of care under a physician’s plan.

Responsibilities
  • Initiate, develop, and implement nursing plans of care and treatment, evaluating patient progress towards goals.
  • Organize and participate in the provision of direct patient care, performing treatments, administering medications, and educating patients and families/caregivers.
  • Modify treatment plans in response to changing patient status or physician orders to achieve established or revised patient care goals.
  • Assess patient needs and physical status at each skilled visit through health data access and patient interview, and reevaluate through subsequent physical reassessments and supplemental physician orders.
  • Obtain and clarify physician orders for plan of treatment revisions; inform physicians promptly of significant changes and provide written summary within supplemental orders.
  • Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.
  • Communicate effectively and timely with clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.
  • Prepare clinical documentation in real‑time during visits and submit to the nursing department, complying with established timelines for billing optimization.
  • Coordinate discharge planning, prepare discharge summaries with patient instructions, and report patient care needs, progress, and goals when transferring care.
  • Demonstrate compliance with agency operations, company policies, professional standards, and applicable regulations and accreditation standards.
  • Maximize work efficiency through the use of computers and automation technologies to validate plan of treatment orders and follow assigned visit schedules.
  • Respond promptly and appropriately to patient requests, initiating emergency procedures as necessary.
  • Maintain ethical and professional conduct, adhering to legal and ethical guidelines for confidentiality and HIPAA regulations.
  • Speak knowledgeably about company services and instruct patients on related financial obligations.
  • Attending and completing required training modules, in‑services, and continuing education to maintain competency and professional licensure.
  • Provide oversight and coordination of paraprofessionals in the home setting, and act as supervisor in the absence of the supervisor or nurse manager as applicable.
  • Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities.
  • Participate in nursing department on‑call responsibilities and perform other related duties as directed by supervisor.
  • When applicable, insert PICC/Midlines per physician order, troubleshoot/clarify obstructed central line catheters, monitor PICC/Midline, and educate on prevention of line infection and routine care.
Qualifications
  • Active and unrestricted RN license in the state of practice.
  • Minimum of 1 year of nursing experience.
  • Current CPR certification required.
  • Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs.
  • Advanced certification and training as applicable.
  • Required licensure to operate a motor vehicle in the state of practice, with access to a vehicle for business travel and proof of liability insurance.
  • Basic knowledge of computer operating systems and software applications with ability to self‑evaluate for annual appraisal and set professional growth goals with nurse manager.
  • Ability to use nursing technology tools to communicate and document care provided.
Physical Demands

Ability to lift up to 50 pounds (maximum 75 pounds), exert up to 20 pounds of force occasionally or up to 10 pounds frequently. Tasks may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.
