Compensation: $125,000 – $150,000/year

Schedule:
Four day shifts (8:30 AM – 4:30 PM) and one evening shift (12:00 PM – 8:00 PM)
TAG Med Staffing is hiring a Family Nurse Practitioner (Primary Care) in New York, NY with our client. In this role, you will deliver comprehensive primary care across the lifespan, performing evaluations, diagnoses, treatments, and follow-up in a team-based setting. You will manage visits, coordinate referrals, document in EPIC, and support quality initiatives. Experience in community health, LGBTQ+ care, HIV care, or underserved populations is strongly preferred.
Responsibilitiesas the Family Nurse Practitioner (Primary Care)
- Comprehensive Clinical Care: Conduct patient assessments, physical exams, diagnoses, treatment planning, and chronic disease management across all age groups.
- Primary Care Management: Provide acute, chronic, and preventive care services including prescriptions, vaccinations, and ongoing monitoring.
- Care Coordination: Follow up on labs, imaging, referrals, and treatment changes to ensure continuity and high-quality patient care.
- Documentation & EMR Use: Maintain accurate, timely clinical documentation in EPIC.
- Team Collaboration & Quality Initiatives: Work within a team-based care model and participate in quality improvement efforts and ongoing professional development.
- Education: Master’s degree in Nursing and completion of an accredited Family Nurse Practitioner program required.
- Licensure: Active New York State NP license required; DEA registration required.
- Experience: 2+ years of primary care experience required, with strong preference for experience serving LGBTQ+, HIV, transgender, or underserved populations.

Proficiency with EPIC or similar EMR systems preferred. - Skills & Attributes: Compassionate, culturally competent clinician with strong assessment and communication skills who thrives in a fast-paced, team-based care setting.
