In-Home Traveler Nurse Practitioner or Physician Assistant - VA

Signify Health, part of CVS Health, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to perform In‑Home Health Evaluations. The clinician will travel up to 100 % of the time across a defined coverage area, requiring overnight hotel stays, car rentals or flights as needed. Multiple state licenses must be held. The role involves building trusted relationships, providing comprehensive health evaluations in patients’ homes, and using an intuitive clinical workflow on an iPad.

Pharmacologic prescribing, lab ordering and treatment modifications are not within scope.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical examination, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) ordered by the patient’s health plan. Interactions occur one‑on‑one, and virtual visits may also be conducted.

As an in‑home clinician, your primary workplace is the private residence of health plan members. Expect diverse conditions that may include household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform duties safely in such environments.
