In-Home Health Clinician - Nurse Practitioner or Physician Assistant; FullTime- Grand Junction

Position

Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and diagnostic tests such as diabetic eye exams or spirometry as ordered by the patient’s health plan.

Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or alter the current treatment regimen.

• Provide all Signify Health services, including diagnostic and preventive services such as spirometry, DEE, bone density screening, etc.

Clinicians work in private residences. Environments may contain pets, secondhand smoke, and airborne allergens. Candidates must be able to perform duties safely in such settings.

Base pay ranges from $87,035 to $187,460 annually, depending on experience, education, geography and other factors.

Primary coverage area:
Grand Junction MSA (Mesa, Delta, Montrose, Ouray, Pitkin, Gunnison, Chaffee).

Anticipated weekly hours: 40. Full‑time position.
