The Staff Pad has partnered with a nonprofit Federally Qualified Health Center (FQHC) in Ontario, CA, to recruit Primary Care Nurse Practitioners who are passionate about delivering exceptional, patient-centered care in a collaborative environment serving a diverse community.
Reporting to the CMO, the Nurse Practitioner will provide comprehensive primary care services, perform health assessments, diagnose and treat acute and chronic conditions, prescribe medications, and collaborate with patients and care teams to deliver high-quality, compassionate care.
Schedule:
Full-Time | 36 Clinical Hours + 4 Administrative Hours
- Monday: 8:00 AM – 9:00 PM (12 hours)
- Tuesday:
Off (or 4 remote administrative hours after onboarding) - Wednesday: 8:00 AM – 9:00 PM (12 hours)
- Thursday: 4 hours of administrative time (may be completed on Tuesday or Thursday)
- Friday: 8:00 AM – 9:00 PM (12 hours)
- Saturday:
Off
- Provide comprehensive primary care for patients of all ages (including children)
- Perform health assessments, physical exams, and develop treatment plans
- Partner with patients and families to achieve self-management goals
- Prescribe medications and order diagnostic tests
- Coordinate specialty referrals and follow-up care
- Maintain accurate EHR documentation and ensure HIPAA compliance
- Submit appropriate billing codes and clinical documentation
- Participate in safety, quality improvement, and required training initiatives to support clinical excellence
- Graduate of an accredited Nurse Practitioner program (Master's preferred)
- Current unrestricted California RN and Nurse Practitioner license
- 2+ years of Nurse Practitioner experience; 1+ year of FQHC experience preferred
- DEA & CPR (for healthcare providers) certifications required
- Experience with Electronic Health Record (EHR) systems
- Bilingual English/Spanish & strong communication skills
