Position Summary
RN Patient Flow Coordinator at LCMC Health—UMC Campus. Responsibilities include coordination and communication of transfer requests for LCMC Health Adult Hospital Market (AHM) facilities using problem‑solving, critical thinking, and excellent customer service skills. Acts as facilitator and decision‑maker applying nursing knowledge for placement decisions and communications. Also responsible for bed planning and throughput facilitation at designated LCMC Health Adult Hospitals. This description outlines the general nature and level of work performed by employees in this role.

- Serve as the “point person” to facilitate incoming transfers/transports and maintain communication with all parties, including but not limited to attending MDs and referring facilities.
- Use problem‑solving skills to determine best course of action that satisfies patients, referral health care teams, and optimizes communication with the appropriate physician.
- Utilize nursing knowledge and education to gather accurate clinical data ensuring appropriate and prompt transfer and placement of the patient.
- Document, track, and coordinate logistics for hospital transfers and bed planning.
- Establish and maintain a high degree of awareness and knowledge of hospital’s medical staff, services, and programs.
- Maintain professional and technical knowledge through continuous education, accept new assignments, and perform additional duties as requested.
- 3 years experience in inpatient nursing, call center, critical care, or ED.
- American Heart Association Basic Life Support/CPR.
- Licensed Registered Nurse through the Louisiana State Board of Nursing.
- Basic nursing skills with high level critical thinking ability.
- Excellent customer relationship skills, including telephone and personal communication with physicians, administrators, nurses, case managers, and other team members.
- Proficient in hospital computer systems for accurate data collection, monitoring, and evaluation.
