We are a multi-specialty private physician group seeking a dedicated and motivated Nurse Practitioner to join our growing medical team on a full‑time basis
.

• Active New York State Nurse Practitioner license required
• Family Medicine, Internal Medicine, Geriatrics, Rehabilitation, or Post‑Acute Care experience preferred
• Previous experience in skilled nursing, rehabilitation, long‑term care, or assisted living is a plus
• Strong communication, clinical judgment, and organizational skills
• Willing to train the right candidate

• Coordinate patient care with attending physicians, families, nursing staff, and facility teams
• Communicate regularly with physicians regarding changes in patient status and treatment plans
• Support the medical team in providing high‑quality, patient‑centered care
• Participate in care coordination, documentation, and clinical follow‑up
• Help ensure continuity of care across post‑acute and long‑term care settings

Schedule: Full‑time schedule

• Nurse Practitioner: 1 year preferred
• Rehabilitation, skilled nursing, or geriatric care experience preferred
