Ritemed is seeking a full-time Nurse Practitioner to provide on-site clinical services at Throgs Neck Rehabilitation & Nursing Center in the Bronx.
This is a facility-based role within a skilled nursing environment. The Nurse Practitioner will manage post-acute and long-term care residents, working closely with attending physicians and facility leadership to support strong clinical outcomes and quality care.
Key Responsibilities- Provide comprehensive medical care to residents within the nursing home
- Perform assessments, follow-ups, and manage acute changes in condition
- Collaborate with physicians and nursing leadership
- Participate in care planning and interdisciplinary meetings
- Ensure timely and accurate clinical documentation
- Support initiatives to improve quality measures and reduce rehospitalizations
- Active New York Nurse Practitioner license
- Minimum of 6 months of Nurse Practitioner experience required
- Nursing home experience required
- Spanish-speaking preferred
- Strong clinical assessment and decision-making skills
- Full-time position
- Shift options: 9:00 AM – 5:00 PM or 3:00 PM – 11:00 PM
