Position Summary

We have an exciting opportunity to join our team as an APP Manager – PA or Certified Midwife.

Job Responsibilities
  • Perform all relevant clinical functions based on evidence‑based approaches to clinical care, using established guidelines. Perform complete health history, physical examination and psychosocial assessment within the designated practice.
  • Work collaboratively with the practice team, attending physicians, house staff, consulting physicians, and other disciplines to identify, develop, implement, and modify plans of care that consider potential outcomes and goals of care, maximize patient/family satisfaction, and enhance quality, access, and cost outcomes.
  • Order laboratory, radiological, and diagnostic studies as indicated, differentiate between normal and abnormal findings, and follow protocols for abnormal findings, adjusting the plan of care or discussing with attendings and house staff as required.
  • Prescribe medications to patients according to NYS law, including controlled substances with DEA authorization. Prescribe medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Evaluate patient responses to treatment and modify per clinical standards and practice protocols.
  • Coordinate discharge planning with interdisciplinary team, patient, and family to assure timely and safe discharge. Practice as a member of the allied health staff according to the rules and regulations, bylaws of the Medical Staff, and within scope of practice and approved delineation of privileges.
  • Act as a professional role model to all levels of staff by incorporating expertise and related experience into patient care. Consider needs and behaviors of specific patient age and cultural groups in all patient care activities.
  • Evaluate advanced practice activities that contribute to achievement of patient outcomes and add value to clinical care. Utilize practice‑based evidence to evaluate care and identify the need for changes in structure or processes to improve care.
  • Participate in the development and monitoring of patient outcomes per established practice protocols for quality and performance improvement. Use evidence‑based health‑care literature and specific department clinical and escalation algorithms to advise and support appropriate practice changes within the designated service.
  • Participate in Service Line Quality and Performance Improvement meetings. Participate in Advance Practice meetings and committees.
  • Assist and participate in the preparation, implementation, and evaluation of research protocols when applicable. Monitor and trend the cost‑effectiveness of the position, especially as it relates to efficiency and performance indicators established for the service.
  • Collaborate with the Sr. Director of Advanced Practice Providers and/or Director of Advanced Practice Providers and the division chief of the clinical department in the recruitment, retention, and human‑resource management of the nurse practitioners, midwives, and physician assistants assigned to the respective service. Guide and monitor performance and delivery of care on a daily basis, including time scheduling, assignments, and coordination of services to patients.
  • Facilitate onboarding and orientation as well as ongoing professional development of nurse practitioners, midwives, and physician assistants. Coordinate clinical activities to accomplish established safety and quality measures. Provide leadership and develop effective team building.
  • Develop and maintain policies, standards and processes pertaining to all aspects of patient care. Set departmental goals in collaboration with division chief and other key personnel. Participate in relevant hospital‑wide committees. Act as liaison between inpatient and outpatient areas to assure continuity of care.
  • Participate in performance appraisals of nurse practitioners, midwives, and physician assistants on the assigned service. Participate in evaluations of fellows, residents, and interns as needed.
