Geriatric And Palliative Care, Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to deliver high-quality primary and palliative care services to our senior residents. Located on a senior living campus known for providing exceptional, independent living, assisted, living, and memory care services. • Provide comprehensive primary care and palliative care to residents in collaboration with our interdisciplinary team.
• Conduct routine health assessments, manage acute and chronic conditions, and prescribed medications as needed.
• Develop and implement individualized treatment care plans.
• Provide health, education and care strategies to promote patient’s aging in place.
• Maintain accurate and up-to-date medical records.

• Current Nurse Practitioner license in New York State.
• Certification in Adult or Adult-Geriatric Preferred.
• Experience in primary care, Geriatrics, or palliative care is strongly preferred.
• Must have a minimum of 3000 hours of clinical experience to be qualified to work as an independent Nurse Practitioner.
• Strong computer skills, ability to adapt and learn a new EMR quickly, excellent communication and interpersonal skills.
• Commitment to compassionate, resident centered care.
• Ability to manage multiple chronic conditions in a community-based setting to prevent acute hospitalizations or transitions to higher level of care.
