Location: New York

Role Overview

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves engaging with patients in their homes, conducting comprehensive assessments, and offering diagnostic and preventive services.

Responsibilities
  • Provide in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical evaluation, and any ordered diagnostic tests.
  • Use an iPad and an intuitive clinical workflow for each evaluation.
  • Bring a compassionate, patient‑centered approach to every visit.
  • Build trusted relationships with patients to improve health outcomes.
  • Be flexible to travel locally and within licensed states and conduct virtual visits as needed.
  • Provide all Signify Health Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams).
  • Safely perform duties in diverse home environments that may contain pets, secondhand smoke, and potential allergens.
Qualifications
  • Active, unrestricted NP or PA license(s) in the coverage area(s).
  • Board certification required.
  • Multi‑state license(s) preferred.
  • Ability to cover primary locations:
    Cambridge, New Hampshire.
Compensation & Hours
  • Part‑time
  • Anticipated weekly hours: 29.5
  • Pay range: $41.85 – $90.13 per hour (base hourly rate or base annual full‑time salary).
