Are you a compassionate and experienced Nurse Practitioner looking for a flexible role that allows you to make a direct impact in patients' lives? We're seeking a dedicated Family Nurse Practitioner (NP) to provide in-home assessments and care planning services throughout the NYC.
Assignment Details- Position:
Family Nurse Practitioner (NP) - Location:
NYC (Hybrid Field-Based Role) - Schedule:
Flexible, Minimum 9 Hours Weekly - Initial Patient Visits:
Approximately 2.5 Hours Each
- Active New York Nurse Practitioner License
- Active NY Medicaid Provider enrollment
- ANCC certification
- NPI number and malpractice insurance
- Minimum 2 years (or 3,600 hours) of NP experience in:
- Family Health
- Adult Health
- Acute Care
