In-Home Traveler Nurse Practitioner or Physician Assistant - WV

Full Time Clinician (Nurse Practitioner or Physician Assistant)

Full‑time clinician to provide in‑home health evaluations for Signify Health, traveling 100% of the time within a defined coverage area. Clinicians will build trusted relationships, conduct comprehensive evaluations, and provide preventive services with an intuitive iPad workflow.

• Perform 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and ordered diagnostic tests.
• Use iPad and clinical workflow to complete visits; not required to prescribe medicine or order lab tests.
• Travel locally and within licensed states, including overnight stays, car rentals, or flights as needed.
• Conduct virtual visits when appropriate.
• Provide all Signify Health services, including diagnostic and preventive services (spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in the coverage area(s). Multi‑state licensing as required.
• Board certification (required).
• Ability to cover all counties within state and flex to other business‑needed areas.
• Full‑time availability, 40 hours per week.

• Experience as NP or PA.
• Ability to obtain and maintain multiple state licenses.

Clinicians will work in private residences, which may contain pets, secondhand smoke, or allergens. Candidates must be able to safely perform duties under these conditions.
