LPN Licensed Practical Nurse Responsibilities- Provide direct patient care, including administering medications, wound care, and vital signs monitoring
- Develop and implement individualized care plans in collaboration with other healthcare professionals
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
- Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
- Collaborate with other departments, such as therapy and dietary, to provide comprehensive care
- LPN license in the state of New York
- 1 year of experience in long-term care or related field preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
