Schedule: Monday - Friday 9am-5pm
Compensation: $110,000 - $120,000
Must reside in NY, NJ, or CT.

Your Day as a Nurse AssessorAs a Nurse Assessor at Village Care, you will play a vital role in ensuring the health and well‑being of our members through comprehensive assessments using the NYS Department of Health Universal Assessment System (UAS). Your responsibilities include:
- Conducting in‑person assessments, evaluating clinical, social, and individual needs to create tailored care plans.
- Collaborating with the assessment scheduling team to confirm assessment sessions and engaging with members, families, and providers to gather crucial information; providing essential self‑care training and ensuring members have access to valuable resources.
- Documenting assessments promptly and working closely with the Quality Manager and UAS Nurse Reviewer to enhance assessment accuracy.
- Participating in quality improvement initiatives to elevate service delivery, ensuring members and their care teams are satisfied and supported in their journey toward health and independence.
To thrive as a Nurse Assessor at Village Care, you must possess a blend of education, experience, and technical skills.
- Minimum of an Associate’s degree in nursing; a Bachelor of Science in Nursing (BSN) is preferred.
- Active NYS RN license and valid NYS identification.
- At least two years of clinical experience, ideally in home health care or within a health plan.
- Familiarity with NYS Managed Long‑Term Care (MLTC) and Medicaid/Medicare plans is advantageous.
- Proficiency in the NYS Department of Health’s Uniform Assessment System (NYS‑UAS) preferred; training will be provided.
- Strong analytical and problem‑solving skills; proficiency in standard office software (MS Word, Excel, Outlook, PowerPoint).
- Familiarity with electronic health record systems; ability to communicate in languages spoken by members, such as Spanish or Mandarin, is a plus.
