Nurse Practitioner

Our client is a growing healthcare organization that provides patient-focused primary care services to residents in assisted living facilities throughout the Albany, New York region.

Our client is committed to delivering high-quality, compassionate healthcare to aging populations. Their team focuses on improving patient outcomes through personalized, comprehensive primary care services provided directly within assisted living communities.

Our client is seeking an experienced Nurse Practitioner to provide primary care services in assisted living facilities throughout the Albany region.

Part-Time Option

  • 2 – 3 Full Days per Week
  • Travel Between Hoosick Falls, NY and Cambridge, NY

OR

Full-Time Option

  • 4 – 5 Full Days per Week
  • Travel Between Hoosick Falls, NY, Cambridge, NY, and Troy, NY

Requirements:

  • Master's Degree in Nursing from an Accredited Nurse Practitioner Program
  • Active New York State Nurse Practitioner License
  • Active DEA Registration
  • Currently Enrolled and Credentialed with Medicare and Medicaid (Required)
  • Ability to Practice Independently (Minimum 3,600 Clinical Practice Hours in Accordance with NYS Requirements)
  • Primary Care Experience Required
  • Strong Clinical and Communication Skills

Preferred Qualifications:

  • Geriatric Experience
  • Assisted Living Experience
  • Long-Term Care Experience

Responsibilities:

  • Monday – Friday Schedule
  • No Nights
  • No Weekends
  • Conduct comprehensive health assessments, physical examinations, and medication reviews
  • Develop and implement individualized treatment plans
  • Manage chronic diseases and acute medical conditions
  • Coordinate care with facility staff, specialists, and family members
  • Provide preventive care and health education
  • Maintain accurate and timely clinical documentation
  • Ensure compliance with Medicare, Medicaid, and regulatory guidelines
  • Additional Duties as Required
