Position Overview:

As an LPN in our skilled nursing home facility, you will play a crucial role in providing high-quality care to our residents. Your responsibilities will include administering medications, monitoring patient health, assisting with treatments, and collaborating with the multidisciplinary team to ensure the well-being and comfort of our residents.

Qualifications:

• Valid LPN license in the state of New York
• Compassionate and empathetic attitude towards seniors
• Strong communication and teamwork skills
• Ability to adapt to various situations and prioritize effectively
• Experience in a skilled nursing home or long-term care setting preferred

Key Responsibilities:

• Administer medications and treatments as prescribed by physicians
• Monitor and document resident health status and communicate changes to appropriate team members
• Assist with activities of daily living and ensure residents' comfort and safety
• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement care plans
• Maintain accurate and up-to-date resident records and charts
• Provide emotional support to residents and their families
