Full time Hospice RN Case Manager role available in Queens NY.

RN will be assigned to a very small territory consisting of 3 zones or zip codes clustered nearby their primary residence (RN will NOT cover the whole borough).

Territory: QUEENS - Field / Community based

Work Schedule:

Monday through Friday 8:30 AM to 5:00 PM

Responsibilities
  • See four hospice patients per day and manage a caseload of terminally ill, end‑of‑life patients, delivering skilled nursing care in home and facility settings, supporting patients and families throughout the end‑of‑life process.
  • Create and follow a plan of care to provide patients with maximum level of comfort and quality of life; responsible for overall case management of assigned patient caseload through coordination of hospice services as determined in the Hospice Plan of Care.
  • Provide physical and psychosocial health system assessments for the purpose of developing the Hospice Plan of Care and goals of care based on the patients’ and families’ needs and wishes.
  • Follow infection control standards as per company policy.
  • Educate patients and families on disease progression, symptom management, and medication administration.
  • Collaborate with physicians to regularly titrate medications and update individualized care plans while simultaneously working with the inter‑disciplinary team in developing comprehensive, patient‑centered plans of care.
  • Assess and make recommendations for medical equipment and supplies and evaluate equipment for safe use.
  • Coordinate with office staff or vendor for delivery of equipment or supplies, anticipating need for supplies prior to weekends or holidays, and maintain proper inventory levels in the home.
  • Act as a liaison for GIP/IPU patients in crisis on the hospice inpatient unit when needed.
  • Maintain accurate and timely clinical documentation.
Qualifications
  • Registered Nurse with an active NY State RN license in good standing.
  • At least 1 year of RN experience in hospice or palliative care, or in medical‑surgical or oncology nursing.
  • BSN degree highly preferred.
  • Must own a car.
