A structured family nurse practitioner (FNP) residency program for new graduate APPs will provide a supportive entry into practice. The goal of the program is to eliminate wide variations in on-boarding and improving competency at the entry level.
During the program, the APP will be paired up with a willing preceptor to allow for collaboration and hands-on education. Structured online and in-classroom continuing education modules will help guide clinical decision making, improve documentation, and expand skill set. The FNP resident will complete hands‑on training in the outpatient and inpatient clinics, with various specialists, primary care provider(s) and hospitalist(s).
Goals of the Program- Improve clinical confidence to deliver high quality patient care to the community.
- Increase clinical knowledge through clinical rotations in primary care and various specialty rotations to manage complex patient care.
- Increasing in provider retention and job satisfaction by receiving quality education and training to assist in first year transition as a family nurse practitioner
- Obtains complete or interval patient history
- Performs assessments via observation, interview and examination, as indicated.
- Orders, performs, and interprets diagnostic studies.
- Obtains appropriate laboratory specimens.
- Performs preventative health assessments, screenings, immunizations and care.
- Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources.
- Provides care in the context of promoting age and culture‑appropriate informed choices, shared decision‑making and the right to self‑determination.
- Provides accurate and timely documentation of assessment and patient care plan.
- Assures treatment and/or plan of care are consistent with established protocols as well as patient preferences.
- Assures appropriate consent forms are signed and dated.
- Refers patients to appropriate services that exceed those of the clinic or scope of practice of the practitioner.
- Assists in the maintenance of adequate patient health records.
- Completes rotations with specialty populations in healthcare, most likely in rural and underserved areas.
- Participates in an interprofessional team‑based clinical practice training model.
- Adheres to all organizational policies and procedures.
- Follows established medical and specialty protocols.
- Executes all tasks and behaves in a manner consistent with a culture of compliance, safety, and high reliability.
- Presents behavior supporting the organization's mission, goals, and core commitments.
REQUIRED:
- Graduation from an accredited school of nursing and Family or Adult Gerontology Primary Care nurse practitioner program within the past 18 months prior to the start date of the program.
- Master's degree or higher in Nursing.
PREFERRED:
- Prior experience with electronic health records.
REQUIRED:
- Current NYS License
- National certification from the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
