Monday to Friday, 9:00 AM to 5:00 PM
TAG Med Staffing is hiring a Licensed Practical Nurse (Residential Youth Care) in New York, NY for our client, providing nursing care, medication administration, and care coordination for adolescents in a residential treatment setting. This role will partner closely with clinical and interdisciplinary teams to support the physical and behavioral health needs of youth, maintain accurate clinical documentation, and promote a safe, supportive care environment.
The ideal candidate will bring strong clinical skills, experience working with pediatric, adolescent, or behavioral health populations, and a commitment to delivering compassionate, patient-centered care.
- Health Assessments & Care Planning: Conduct assessments, home visits, and develop individualized care plans for adolescents.
- Medication Management: Administer medications, monitor responses, and maintain regulatory documentation.
- Care Coordination &
Collaboration:
Partner with families, providers, and multidisciplinary teams to ensure continuity of care. - Education & Support: Provide health education to clients and families, promoting independence and self-care practices.
- Trauma-Informed Practice: Utilize Sanctuary Model tools to create a safe, supportive, and recovery-focused environment.
- Education: Graduate of an accredited practical nursing program required.
- Experience: 2+ years of practical nursing experience preferred, ideally in residential care, pediatrics, adolescent health, or behavioral health.
- Licensure & Certification: Active New York State LPN license and current CPR certification (American Heart Association or American Red Cross) required.
- Technical
Skills:
Strong knowledge of nursing principles and regulatory documentation standards, with the ability to manage medical records and coordinate across healthcare providers. - Skills & Attributes: Strong communication skills, collaborative approach with youth, families, and staff, commitment to trauma-informed care, and flexibility to accompany adolescents to appointments.
