In-home Traveler Nurse Practitioner or Physician Assistant Massachusetts

Full‑time Clinician (Nurse Practitioner or Physician Assistant) – In‑Home Health Evaluations. Provide care and health assessments in patients’ homes, traveling up to 100% of the time across a defined coverage area.

• Travel locally and within licensed states up to 100% of the time, including overnight hotel stays, car rentals and/or flights as required.
• Conduct in‑home visits lasting approximately 45 minutes, which include a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) when ordered by the patient’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; you are not required to prescribe medicine, order lab tests or alter the patient’s current treatment regimen.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
• Conduct virtual visits as needed.
• Build trusted relationships to improve patient health and connect patients to the right care for their unique needs.
• Assist with per‑completion incentive pay for all in‑home visits.
• Follow safety and quality standards while working in private residences that may contain pets, secondhand smoke or allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses in coverage areas as needed.
• Coverage of all counties within Massachusetts with flexibility to other business‑required areas.
• Minimum of 40 anticipated weekly hours.
