In-Home NY Traveler, Nurse Practitioner or PA

Position

Full‑time Clinician (Nurse Practitioner or Physician Assistant) – In‑Home Health Evaluations.

Join Signify Health, part of CVS Health®, to provide home‑based health evaluations and bridge gaps in care. You will travel up to 100% of the time across a defined coverage area, may require overnight stays, and must hold multiple state licenses. You’ll build trusted relationships, provide evaluations, and may conduct virtual visits as needed.

• Travel locally and within licensed states up to 100% of the time.
• Conduct in‑home visits lasting about 45 minutes, including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and intuitive clinical workflow; not required to prescribe medicine, order lab tests or alter treatment regimens.
• Provide Diagnostic & Preventive Services products (spirometry, DEE, etc.).
• Conduct virtual visits as needed.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses as required.
• Coverage: all counties within New York and flex to other business‑needs areas.
• Full‑time commitment, 40 hours per week.

Primary workplace is member’s residence; must be comfortable and safe performing duties in homes that may contain pets, secondhand smoke and allergens.

The typical pay range for this role is $95,738.00 - $, based on experience, education, geography and other factors.
