Come lead with us at New York, NY. $15,000 Sign‑on Bonus or Student Loan Assistance! The position will cover Manhattan with coverage for Brooklyn when needed. The role is to provide comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self‑management education, and tools for patients to better manage multiple conditions.

About the Organization

MJHS is a large not‑for‑profit health system in the Greater New York area. It offers home care, hospice and palliative care, rehabilitation and nursing care at Menorah and Isabella Centers, and research‑based innovation. MJHS Medical Associates is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who reside in assisted living, long‑term care facilities, or at home.

Responsibilities
  • Ensure development of care planning and access to preventive care services in accordance with member and plan health outcome goals and objectives.
  • Provide and/or coordinate primary clinical care services for assigned nursing home residents.
  • Act as a clinical resource to participating providers and facilities.
  • Act as a resource and contact for member or designated representative regarding all aspects of the member’s individualized care plan and disease process.
Qualifications
  • Master’s degree in nursing, graduate of an accredited Nurse Practitioner program required.
  • One year of clinical experience as a nurse practitioner required.
  • Minimum one year in geriatric specialty.
  • Clinical RN/NP experience in acute, subacute and/or long‑term care setting preferred.
  • Prior experience in managed care a plus.
  • Ability to assess, diagnose, treat and follow‑up.
  • Ability to work independently and as part of interdisciplinary teams.
  • Ability to develop relationships and communicate with members/family.
  • Establish priorities and maintain schedules.
  • NYS NP license, registration, and DEA certificates.
  • Valid driver’s license and reliable car required.
