We are seeking an experienced, compassionate, and highly professional Concierge Homecare Registered Nurse who is inspired by our mission and energized by delivering elevated, personalized care in home environments.

Role Overview

As a Concierge RN with Renewal Memory Partners, you will provide premium, high-touch nursing care to clients in their homes, focusing on comprehensive assessments, proactive clinical oversight, and supportive guidance for clients and families navigating cognitive change.

This is an ideal role for an RN who values autonomy, meaningful client relationships, and a service experience free from the time pressures of traditional clinical settings.

Location: New York City & surrounding areas

Schedule: Flexible per diem — based on your availability

Key Responsibilities
In-Home Clinical Care
  • Perform comprehensive intake assessments, including medical history, medication review, cognitive screening, functional evaluation, and psychosocial and environmental assessments
  • Conduct ongoing reassessments and monitor changes in cognitive and physical status
  • Provide hands‑on clinical care such as wound care, medication reconciliation, and symptom monitoring
  • Develop and maintain personalized care plans aligned with each client’s goals and clinical needs
  • Deliver education and coaching to clients and families on dementia progression, symptom management, communication strategies, and daily care techniques
  • Serve as a reliable, proactive clinical liaison to clients, families, and care teams
  • Coordinate care with physicians, specialists, and ancillary providers to ensure seamless integration
  • Facilitate timely follow-up on medical orders, provider recommendations, and care plan updates
  • Guide clients and families through urgent situations with a calm, supportive, and clear approach
  • Maintain the responsive, compassionate, and polished communication expected in concierge home-care services
Documentation & Quality
  • Maintain accurate, compliant electronic documentation
  • Support quality assurance processes and periodic reporting
  • Use digital platforms to update records, track care tasks, and communicate with leadership
Qualifications

We are looking for nurses who embody both clinical excellence and a relationship-centered approach. Ideal candidates have:

  • Active RN license in New York State
  • At least 2 years of nursing experience (home health, geriatrics, dementia care, hospice, or concierge care preferred)
  • Strong clinical assessment and decision‑making skills
  • Experience supporting clients with Alzheimer’s disease or related dementias
  • Exceptional interpersonal and communication skills
  • Tech‑savvy comfort with EMRs, apps, and virtual communication
  • Current BLS/CPR certification
  • Warmth, empathy, and professionalism in every interaction
