Nurse Practitioner

We are recruiting on behalf of a confidential client for an experienced Nurse Practitioner to join a specialized care team serving frail and elderly patients across nursing home and community-based settings in Erie County, NY. This is a Monday through Friday role with a daytime schedule, offering stability, meaningful work, and a compensation package that stands out.

Conducting assessments, developing and managing care plans, and collaborating with an interdisciplinary team to coordinate care for a vulnerable patient population enrolled in an Institutional Special Needs Plan (ISNP).

• Base salary of $120,000 to $135,000
• $10,000 sign-on bonus
• $10,000 annual performance bonus
• Up to 4 weeks PTO to start, with growth over time, plus 9 paid holidays

A licensed Nurse Practitioner in New York with prior clinical experience. A background in geriatrics or managed care is a plus. Strong communication skills and a team-oriented mindset matter most here.
