Job Title:
Associate Director of Nursing - Care Management
Location:
New York, NY 10037 Duration: 3+ months (Possible extensions or FT Conversions)
Shift: Days | 9:00 AM - 5:30 PM
Schedule:
5 days/week, 8 hours/day, 40 hours/week, No On-Call Pay Range: $100 - $105/hr. on W2 Schedule Notes:
- CCM & PRI Certification is mandatory.
- Provide Associate Director level supervision to the Care Management Department.
- Work Setting: Care Management / Transitional Care
Skills & Experience:
- 4+ years of Case Management experience in a large Acute care facility.
- Seven (7) years of nursing experience.
- Five (5) years of supervisory experience.
- Minimum two (2) years of discharge planning experience.
- Minimum two (2) years of utilization review experience.
- Care management experience.
- Assists with the planning, organization, direction, coordination, and evaluation of nursing services and educational programs within the Transitional Care Division of Care Management, with a focus on reducing 30-day readmissions and supporting high-risk patients transitioning from inpatient and emergency department settings to the community.
- Assist with planning, organizing, directing, coordinating, and evaluating nursing services and educational programs.
- Perform duties of the Director of Nursing in their absence.
- Participate in developing policies and standards of nursing service and education within Transitional Care.
- Provide oversight and direction to Accountable Care Managers (RNs), Care Coordinators, clerical staff, and ancillary personnel.
- Monitor patient care performance and employee performance.
- Assess units with nurse managers and assist with corrective action plans.
- Develop programs to improve patient care outcomes and employee performance.
- Develop and maintain record-keeping systems related to medication errors and counseling activities.
- Collaborate with the Transitional Care team to reduce 30-day readmissions.
- Support high-risk patients following discharge from inpatient Med/Surg units and the Adult Emergency Department.
- Coordinate community-based care management services to optimize patient outcomes after discharge.
- Organize and oversee care coordination activities supporting safe transitions to community settings.
- Bachelor's Degree in Nursing from an accredited Nursing Program.
- Master's Degree in Nursing.
- RN License.
- Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
