The primary function of the LPN is to assist with assessment, planning, implementing, and evaluating the care of all patients during their shift. Responsible for meeting the established standards of skilled nursing practice. Collaborates with the care team and participates in interdisciplinary care. Advocates for patients and effectively provides person-centered care and services. Administers medications and treatments under the supervision of a registered nurse in accordance with facility policy.
Takes patients' vital signs such as respiration, temperature, blood pressure, and heart rate, and reports changes in patients' conditions. Practices ethically and professionally within their scope and advocates for patients. Maintains clinical competence specific to their care area, including any required education. Coordinates care with the interdisciplinary team and documents appropriately and thoroughly in the electronic medical record. Keeps nursing personnel and the responsible registered nurse informed regarding changes in the condition of patients and other pertinent observations.
Availability to work every other weekend and every other holiday is required. May be required to float to other units as staffing permits and may be needed to drop to a CNA position as staffing needs arise. This position involves a staffing ratio of 2 LPNs to 3-4 CNAs for 40 residents. Scrubs of any color may be worn by the employee.
