This field-based position will cover the following locations:
Manhattan, Bronx, Brooklyn, Queens, and Long Island
Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life‑limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life‑limiting illness or end‑of‑life care.
Responsibilities- Provide NP‑appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care.
- Work in the community environment, and may, on occasion, provide consultation in the long‑term care environment or hospital.
- In all settings, provide in‑person visits or telehealth visits, as appropriate.
- Provide comfort care consistent with best practices, including history‑taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation.
- Coordinate with the team physician and manager, and be responsible for face‑to‑face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care.
- Contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed.
- Help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross‑coverage for when young adolescents are incorporated into adult hospice teams.
- Use an evidence‑based approach to management and to the use of materials and policies/procedures when providing both direct care and support to other clinicians.
- Other activities related to quality improvement projects may also be required, as appropriate.
- Master’s degree in nursing and graduate of an approved Nurse Practitioner Program required.
- Minimum of 1‑year clinical experience as an NP provider preferred.
- Prior experience working as a member of a specialist‑level palliative care team or working in a hospice environment preferred.
- Prior pediatric nursing or pediatric NP experience strongly preferred.
- NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner) required.
- BLS required.
- NYS Driver's license and car.
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire.
- Microsoft Word and Excel proficiency.
- Ability to work flexible hours to meet the needs of the position.
- Ability to coordinate resources and personnel involved in a comprehensive interdisciplinary team environment.
- Ability to travel across the agency’s geographic territory to provide patient care as needed.
- Teaching, communication, and listening skills.
- Ability to work non‑judgmentally with patient/family members of any culture, religion, socio‑economic background, or lifestyle.
- High degree of accountability.
- Excellent communication, listening, and organizational skills.
- Ability to work within an integrated health care team.
- Effective critical thinking skills.
- Ability to problem‑solve independently.
