About the RoleDaVita seeks a Clinical Coordinator to lead nursing care delivery in an outpatient dialysis center, coordinating the care of end‑stage renal disease (ESRD) patients, tracking lab values, clinical metrics, and ensuring patient safety and quality outcomes.
Responsibilities- Coordinate ESRD patient care and services, trend lab values, track metrics on vital signs, weight, and outcome management.
- Deliver outstanding clinical outcomes and improve patients’ quality of life in a chronic hemodialysis setting.
- Build long‑term relationships with patients and their families.
- Lead and support the clinical team, fostering a collaborative work environment.
- Contribute to quality improvement initiatives and help maintain DaVita’s high standards for care.
- Current Registered Nurse (RN) license in the state of practice.
- Current CPR certification.
- At least 12 months of registered nursing experience.
- At least 6 months of dialysis experience.
- Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN‑licensed FA/GFA.
- Associate Degree in Nursing (ADN) from an accredited nursing school.
- Completed training approved by the medical director and governing body as required by CMS guidelines prior to operating the water treatment system.
- Willingness, desire, and ability to supervise.
- Basic computer skills and proficiency in MS Word and Outlook.
- Functional proficiency in all DaVita computer systems within 90 days of hire.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
- Experience in ICU, CCU, ER, or Medical‑Surgical units.
- Bachelor of Science in Nursing (BSN) or a three‑year accredited diploma program may be substituted.
- Prior supervisory experience.
