About the RoleOptum Home & Community Care, part of the Optum family, is building an integrated care model that holistically addresses patients’ physical, mental, and social needs. The Senior Community Care (SCC) program delivers clinical care to high‑risk health plan members in nursing homes, senior housing, and assisted living settings. As a Senior Community Care clinician, you will coordinate care, support patients through short‑term sub‑acute rehabilitation episodes, and engage families in care planning.
PrimaryResponsibilities
- Conduct comprehensive assessments:
- Past medical history review, medication reconciliation, vital signs, and physical exam
- Evidence‑based screenings and point‑of‑care testing (as appropriate)
- Support members during a short‑term sub‑acute rehabilitation episode
- Ensure accurate and complete documentation, including ICD‑10 codes
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
- For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care
- For PAs: Current NCCPA certification and state licensure
- Active, unrestricted licensure in the state of practice (or ability to obtain by start date) with no disciplinary actions or practice limitations
- Active and unrestricted driver’s license and reliable transportation
- Current, active DEA licensure or prescriptive authority (or ability to obtain) per state regulations, unless prohibited
- Ability to obtain a collaborative practice agreement, if applicable in your state
- Ability to navigate varied care environments and perform job duties as required
- 1+ years of clinical experience as an APC, preferably in long‑term care
- Experience in geriatric medicine, long‑term care, senior living, or home care settings
- Experience meeting medical needs of patients with complex behavioral, social, and/or functional needs
- Understanding of geriatrics and chronic illness management
- Understanding of advanced illness and end‑of‑life discussions
- Proficiency with electronic medical records and technology
- Compensation range: $109,500 – $164,000 (base pay + bonus)
- Total cash compensation varies by local labor market and individual credentials and may increase with performance
