- Position:
Home Dialysis Registered Nurse (RN) - Manage outpatient home dialysis clinics
- Conduct patient assessments and ongoing clinical management
- Provide modality education and training for new home dialysis patients
- Troubleshoot and triage patient concerns and treatment-related issues
- Coordinate care and support patient independence in the home setting
- Occasionally assist inpatient nursing units with CAPD exchanges
- Complete initial home visits for new patients as needed
- Travel between clinic locations in Cooperstown, Oneonta, and Little Falls may be required
- RNs expected to travel to patient homes for initial home assessment/visit
- Ongoing patient care primarily provided in clinic setting
- Agency nurses shadow experienced team member during first week orientation
- After orientation, nurses manage assigned patient population independently
- Preferred previous Home Dialysis experience
- Peritoneal Dialysis (PD) experience highly desirable
- Experience educating and training dialysis patients preferred
- Strong assessment, triage, and patient education skills essential
- Independent and self-motivated clinician
- Comfortable managing small patient panel across multiple clinic locations
- Experienced in troubleshooting dialysis-related issues and providing patient education
- Adaptable and able to work autonomously after brief orientation
- Comfortable participating in rotating on-call schedule
- Role offers opportunity to develop long-term relationships with patients
- Supports patients in maintaining dialysis treatments at home
- Combines clinic-based care, patient education, care coordination, and clinical autonomy within supportive home dialysis program
Job Details
- Previous Home Dialysis experience strongly preferred
- Peritoneal Dialysis (PD) experience highly desirable
- Experience educating and training dialysis patients preferred

- Personal vehicle required for travel between clinic locations
Job Requirements
- Schedule:
4 x 10-hour shifts, 6:00 AM – 4:30 PM - Weekday call coverage: 4:30 PM – 6:00 AM
- Weekend call coverage: 24-hour coverage Saturday and Sunday
- Call rotation typically every other week

- Patient population primarily Peritoneal Dialysis (PD)
- Program supports Peritoneal Dialysis (PD) and Home Hemodialysis (HHD)
- Manage outpatient home dialysis clinics
