Nurse Practitioner; Neurology – SNF

Salary Range: $120,000 - $180,000 per year (commensurate with experience)

Elaya Health is a leading provider of endocrinology and specialty care services in nursing homes. We are expanding our services and looking for a compassionate and experienced Nurse Practitioner (NP) with a neurology background to provide high-quality care to patients in long-term care facilities in New York City.

• Provide neurological evaluations and follow-up care for nursing home residents
• Diagnose and manage conditions such as stroke recovery, Parkinson’s disease, dementia, neuropathy, and other neurological disorders
• Collaborate with nursing home staff, families, and interdisciplinary teams to create comprehensive care plans
• Document all patient encounters and maintain compliance with healthcare regulations
• Educate patients, caregivers, and facility staff on neurological conditions and treatments

• Licensed Nurse Practitioner (NP) in New York State
• Board Certification in Neurology or experience in neurology preferred
• Minimum 1-2 years of NP experience (geriatrics or neurology a plus)
• Experience in long-term care or nursing home settings preferred
• Strong clinical judgment and the ability to work independently
• Excellent communication and documentation skills
