Position Summary

The Clinical Coordinator I/RN will work in the Adult and Pediatric Center for Liver Disease & Transplantation within the Division of Abdominal Organ Transplantation in the Department of Surgery. The position will work under the direction of the Senior Nurse Practitioner, Medical and Surgical Directors of the CLDT and in collaboration with Pediatric Hepatologists, Surgeons, Nurse Practitioners, Physician Assistants who see patients at all sites throughout NY and NJ.

The position will be based at the main campus of CUMC.

Responsibilities
  • Completion, documentation and ensuring compliance of principal components of the laboratory and radiology testing and patient follow-up; providing expertise in management of immunosuppression under the supervision of attending NP and Hepatologist.
  • Support on-going clinical functions of the service to provide compassionate patient-centered care.
  • Coordinate the evaluation process for potential transplant candidates with the multidisciplinary team.
  • Coordinate patient consults for transplant evaluation.
  • List and maintain patients on liver/intestinal transplant waitlist.
  • Participation in administrative, selection and education meetings.
  • Provides data to regulatory agencies.
  • Demonstrates knowledge of compliance with United Network for Organ Sharing (UNOS) policies and listing requirement.
  • Coordinate patient care including an evaluation of diagnostic testing, review of lab reports, and direct patient care under the supervision of the attending NP’s and Hepatologist.
  • Collaborate with the multidisciplinary team members to determine and achieve specific health outcomes for patients.
  • Assess conditions of assigned patients.
  • Identify nursing needs of the patient and prescribed treatment.
  • Facilitate office care of patients and record-keeping.
  • Participate in interdisciplinary rounds or conferences with physicians and others.
  • Facilitate support/education groups in collaboration with other healthcare providers.
  • Assure that the clinical standards relative to the documentation of the patient’s plan of care is explained to the patient and family.
  • Collaborate with treatment team and ensure continuity of care among the professional disciplines make patients’ care as seamless as possible.
  • Address appropriate measures to provide patient and family satisfaction.
  • Assist practitioners with clinical judgment affecting patient care.
  • Serve as liaison between patients, physicians, clinical and research teams, and other healthcare providers.
  • Maintain overview of the patient’s progress through the healthcare continuum; facilitate ancillary testing and referral for services, avoiding duplication of effort.
  • Communicate with related services regarding patients’ needs.
  • Pursue opportunities and develop action plans to improve patient care and resolve identified problems.
  • Assure that nursing care is administered in accordance with safety regulations identified by standards, the institution and regulatory groups.
  • Attend the required mandatory education programs on an annual basis.
  • Assume responsibility for professional competence through demonstrated willingness to evaluate own nursing practice, maintenance of working knowledge of current nursing literature and attending continuing education programs.
  • Performs other related duties as assigned.
Minimum Qualifications
  • Bachelor’s Degree in Nursing or equivalent in education and experience.
  • NYS RN License.
  • 3 Years of Related experience.
Preferred Qualifications
  • Intermediate to advanced level MS Office skills.
