St. Josephs Health is seeking Family Medicine Nurse Practitioners or Physician Assistants to join our primary care team in Fayetteville, NY. This is an excellent opportunity to join a well-established, outpatient practice with strong system support, delivering comprehensive, and patient centered care.

• Provide full-scope primary care services including preventive care
• Treat patients of all ages including pediatric, adolescent, adult, and geriatric populations
• Perform routine physicals, health screenings, and immunizations
• Work within a collaborative, team based outpatient setting

• Well-established, hospital-affiliated outpatient practices
• Strong clinical support and resources across the system
• Physician-led organization with a collaborative care model

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Board Certified NP or PA
• NYS licensure and DEA certification required
• Willing to accept new graduate applicants

• Competitive Salary:$105,000 to $125,000 per year, plus additional productivity bonuses and salary increase with tenure
• The posted salary reflects the starting range of total compensation and does not include productivity bonuses or additional incentives
